For New Registrations:

All checks are made out to HYAL
$150 per Football player
$140 per Cheerleader
$85 per FLAG Football player & Cheerleader
**$52 Box of candy per family or $30 buy out amount
**Please include either amount in with your registration check amount
(you will receive candy at registration)

Additional Checks
$50 Calendar raffle per child
(Family max $100, Checks dated August 15, 2009)
$200 Uniform deposit per child (check dated November 1, 2009)
$200 Work deposit per family (check dated November 1, 2009)

NEW PLAYERS & CHEERLEADERS
Birth Certificate
Proof of Residency (gas, electric, or cable bill)
Updated Picture of Child
All participants must have the official POP Warner Physical form
filled out prior to August 1st 2009 or the player WILL NOT be
allowed on the field. This is the ONLY form that will be accepted.
